Registration by Fax

1. Download and fill in the Toronto MELAB Centre Application Form (click here) .
2. Fill in the credit card information section (Visa and MasterCard only).
3. Fax your application to (416) 946-0337.
4. You will receive your receipt by email.
5. If you wish to confirm that we have received your fax, please e-mail us at info@melab.ca. We will return your e-mail within two business days.
