Identification (ID)

  • You do not need identification to apply for the test.
  • To write the MELAB test, you must have two pieces of photo identification and a recent photo that looks like you do now.
  • If you do not bring proper ID, you cannot write the test.

1st Piece of Photo ID

Your first piece of ID must be one of the following:

  1. Your driver's license (from any country as long as the information is in English)
  2. Your passport (from any country as long as the information is in English)
  3. Your Ontario Health Insurance card (with your photo on it)
  4. Your Permanent Resident card (with your recent photo)
  5. If you do not have any of these pieces of ID, you must provide an affidavit with your photograph attached, notarized by a lawyer or notary public.

2nd Piece of Photo ID

Your second piece of ID must confirm the information on your first piece. For example, your name must be spelled the same way and be in the same order on both pieces of ID.

Your second piece of ID can be

  1. Any of the IDs listed above if you have more than one
  2. Your student ID card (with your photo on it)
  3. Your volunteer card (with your photo on it)

Photo for Your ID form

Your photo does not need to be an official passport photo. It can be a recent grad photo, digital photo, booth photo, etc. It must be a recent photo, and it must look like you do now.
