ID Form

• When you get your confirmation, you will also receive an Official Identification Form.

• The ID form must be filled in before you come to the test.
• YOU MUST FILL IN THE SAME LAST NAME, FIRST OR GIVEN NAME, AND MIDDLE NAME that you wrote on your application form. BE CONSISTENT!!
